FMC^® Certificate Overview

The following information shows where you will find certain information on your FMC® Certified Food Protection Manager’s certificate.

1. FMC® Food Managers Certification logo watermarked.
2. The QR code will take you to the Certificate Verification page for certificate validation.
3. Exam information:
   • Exam Completion Date
   • Expiration Date of the Certificate - Certificate Validity is 5 years from Exam Completion Date
   • Exam Form number taken
   • CE hours earned
   • ANAB Provider ID#
4. Candidate Certificate number.
   • The leading alpha characters represent the location the exam was purchased through. They are for FMC® use only.
   • The following series of numbers are the actual unique Certificate number specific to the candidate permit.
5. Candidate name.
